Harm Reduction Case Worker, Open Access Service – Merchants Quay Ireland – Dublin

Objective:
To work directly with clients in developing care plans using a case management approach and delivering harm reduction interventions.
To facilitate low threshold community detoxification in conjunction with partner agencies
To ensure high quality of service is upheld in line with best standards and MQI policies and procedures.

Person Specification
The ideal candidate for this post will have:
A relevant third level qualification in the social care field and at least 2 years’ experience working in a low threshold service
An experience of working with issues relating to care planning and case management regarding alcohol and drug misuse, specifically within a low threshold environment and an awareness of behaviour and social problems affecting them
Experience and understanding of undertaking risk assessments, assessing needs and developing and following through on care plans
Possess strong interpersonal skills and have experience of working in a multidisciplinary/multiagency team within a low threshold environment
Ability to communicate and engage challenging behaviour in a respectful and dynamic way
Experience and understanding of issues relating to drug and alcohol use/homelessness / mental health and undertaking harm reduction interventions as required
Strong knowledge of homeless and drug services and relevant referral pathways is a distinct advantage
Possess a flexible professional approach to their work, and to be open to working in a change environment.
