Roles and responsibilities of the Project Worker
Primary Purpose
To deliver on the actions in CFL under Action 3.1.3(b) and 3.1.3( c) Both these actions are focussing on continuing to deliver, review, and develop the Flourish Social Prescribing which Service currently includes one to one support and a menu of five group interventions which address life transitions and social isolation in Castlebar and surrounding areas. The successful candidate will be responsible for extending the Social Prescribing Service in partnership with the Primary Care Teams, and other local & statutory organisations to Claremorris, Ballina & possibly Westport. The successful candidate will be an employee of the Family Centre, Castlebar.
Main duties include.
To deliver the Flourish Social Prescribing Service in line with all the learning and development over the last eight years and the needs of service users, Primary Care Team, other agencies, and statutory & non statutory bodies.
To deliver this programme under the direction and guidance of the current project worker and the Director of the Family Centre in various towns in Co. Mayo
To meet with potential participants on a one to one basis to support them to access a wide range of social contacts and activities in their area
To receive and process referrals from the general public, PCT and all other appropriate referrals
To build effective working relationships with the PCT and other local organisations
To participate in ongoing evaluation of the work
To keep appropriate records as required by the Family Centre
To comply with the policies and procedures of Family Life Services particularly in relation to confidentiality, quality of work, and Health and Safety
To participate as required in any in-house training
To provide ongoing updated reports and statistics for funders, Board and Annual Report in relation to the work.
Criteria
Relevant third level qualification in the health /community / social area such as social studies, community work, counselling, health promotion or similar or equivalent
Excellent verbal and written communication skills
Experience of dealing with members of the public in particular with people experiencing social or psychological difficulties
Ability to work on own initiative and manage a busy workload.
Previous experience of working in a health or community setting is desirable.
Ability to recognise difficulties with participants which may warrant supervision or referral to an appropriate agency
An understanding of mental health and factors that influence mental wellbeing
Excellent team working and partnership working skills
Ability to build networks with the local community in a short time
Access to appropriate transport
Willingness to be flexible in relation to work demands for flexible working times and days
Possess appropriate Garda clearance
Working Hours: Part Time
Hours per week: 14 and on occasions 21
Gross Annual Salary (include. PRSI & Pension): €22,000
Employment arrangements: The Project worker will work a 2 day per week arrangement, with the possibility of an extra day at various times. We will require flexibility because of the nature of the work and the hours will be14 a week with the occasional extra day (7hours) as needed and by arrangement.
