Summary:
The purpose of the Think Ahead Planning Co-Ordinator role is to support service users to maintain control of all aspects of their life, in particular decisions around their healthcare, future life planning and end of life planning. This role is to ensure that support is implemented in line with the HSE National Consent Policy, Assisted Decision-Making Act (ADMA), and guidance from the Irish Hospice Foundation Think Ahead Planning Pack.
Interviews expected to take place for this vacancy, Week beginning 13th June 2023.
Duties and Responsibilities:
Support the service users to understand the purpose of Future Planning, ensuring all information is available in a format that is accessible to their preferred communication needs.
Complete the Think Ahead Planning Pack with the service user.
Ensure conduct is in line with the Reach Deaf Services Policies and Procedures, specifically GDPR and Confidentiality.
Support the service user to make informed decisions about their future healthcare.
Work as part of a small dynamic team in providing person centered emotional and practical support
Manage and prioritise own caseload and work with flexibility and initiative.
Work with service users across a range of settings including service user homes and other relevant venues.
Accurate record keeping and reporting periodically on progress
Provide guidance only, where required, in making a will or appointing an Enduring Power of
Attorney with the service user
The above list is not exhaustive and you will be required to adapt to the role in line with the agreed vision for the individuals who are supported by Reach Deaf Services in the community.
Knowledge (Education & Related Experience):
Essential:
Fluency in Irish Sign Language (ISL), another signed language or a willingness to participate in ISL classes and achieve ISL level 4 within 18 months of taking up the position. (CIDP will support ISL level 3 and ISL level 4 classes)
Experience working with Irish Sign Language interpreters and Deaf relay interpreters
Possess the interpersonal and communication skills necessary to establish trusting relationships with service users and encourage engagement with the process of future planning
Experience of working within communities with a diverse range of stakeholders.
Ability to research information from a range of sources
Experience of setting and maintaining appropriate boundaries with community members and professionals
Experience and understanding of safeguarding and promoting the welfare of children and vulnerable adults
Desirable:
Experience and understanding of mental health and social care services
Knowledge of Social Role Valorisation
