Duties & Responsibilities:
In this role, the successful candidate will be responsible for the following;
Source Referrals
Support the promotion of the Working Care Out project through the development and dissemination of project resources to include leaflets, posters and regular online webinars.
Liaise with external stakeholders to explain the project and to source referrals.
Engage with media to promote the project.
Delivery of Individual Support to Family Carers
Efficiently and effectively triage and assess family carer needs for supports using the agreed Family Carers Ireland (FCI) triage system and the Carers Outcome Star™ via phone or video call depending on the preferences of the family carer referred to the project.
Develop and agree a person-centred action plan with the family carer based on the Outcome Star™ assessment, with SMART actions agreed to support the family carer.
Assist family carers to access appropriate supports including information, referral to other services, referral to online services (including education), the emergency card scheme, carer counselling, respite, and the Working Care Out virtual clinic and education programmes.
Prioritise family carers into the right services based on the Carers Outcome Star action plan and outcomes.
Organise family carer supports, as required, based on situation and carer needs.
Work with other FCI staff to ensure appropriate supports are put in place.
Complete family carer follow-ups and reviews.
Write case notes, goal and action orientated plans mapped against outcomes.
As required, liaise with family carers and negotiate with service providers to put supports in place for high complex need situations.
Act precisely and calmly when dealing with family carers in high stress situations.
Develop and use knowledge of services and supports offered informally and formally internally and within the community to aid family carers to access appropriate supports and information to address their needs.
Develop, understand and use knowledge of the challenges/stresses facing carers to provide emotional support using a strengths-based approach.
Performance Management
Carry out your position and responsibilities in line with our company values, policies, procedures and processes.

Actively participate in performance management initiatives including performance reviews, objective setting, coaching, career or personal development opportunities and performance improvement plans, as required.

Use effective time management to achieve the key performance indicators (KPIs) of the role.
Practice self-care and open communication to manage emotional carer conversations.
Reporting

Adhere to all internal and Dormant Accounts Funding (DAF) procedures in spending and controlling the Organisation’s funds.
Provide progress reports as required by Pobal and the Organisation.
Keep detailed case notes associated with each family carer who engages in the project.
Systems and Databases
Ensure all relevant organisational databases are kept fully up to date as required by the Organisation. Of particular note is the CRM and the Carers Outcome StarTM database.
Health Safety and Environmental
Ensure the Organisation’s Health and Safety policy and procedures are adhered to and carry out your roles and responsibilities as detailed in the relevant policies and procedures.

Adhere to the Organisation’s policies and procedures.
Other Duties
Undertake other duties as may be required and assigned by the Organisation from time to time.

Role Criteria:
The ideal candidate will have the following qualifications, skills and attributes:
An honours degree (QQI 8 or higher) in the area of Social Work/Community Development/Social Care/Psychology/Supported Employment.
At least 2 years previous experience providing support to family carers and/or those engaged in employment activation in a community-based setting.
Ability to prioritise tasks and work within a dynamic environment.
Excellent IT Skills – mainly MS Word, Excel, Outlook, PowerPoint and experience and confidence using videoconferencing platforms.
Proven networking skills.
Flexibility in attitude and approach to the job.
Reflective approach to their work.
A strong work ethic.
Willingness to do some evening work when required.
Strong facilitation and presentation skills.
Excellent oral, empathic communication skills and the ability to establish rapport with a diverse range of people.
Demonstrated ability to work under pressure, make clear and quick decisions and work with clients in distress.
Effective time management.
Ability to work autonomously and within a team.
Full, clean drivers licence with access to own vehicle.

The following is also desirable:
Experience of using the Outcome StarTM or other assessment framework to determine needs and supports.
Experience of managing projects.
Experience of developing and delivering education programmes.
Experience of working with and responding to client’s individual needs which may include advocacy.
Experience negotiating with community-based organisations or groups to maximise outcome for clients.
Demonstrated ability to apply a person centred or strength-based approach and a consumer directed model of service provision.
Knowledge of challenges facing family carers seeking employment.
Knowledge of services and supports offered formally and informally within the community services sector including within the further education / employment activation sectors.
