Community Support Workers in CRC Transition Programme are responsible for supporting adults in their achievement of individual goals through a Person Centred Planning (PCP) process based on HSE New Directions Policy, as well as providing individualised personal care support as required. Based initially in our centre in Clontarf, the Community Support Workers be working in the northside area of Dublin, supporting adults in achieving their goals in the community.
Candidates for this position should:
Have one years’ experience of working with adults with disabilities is desirable. This should include management of all aspects of personal care.
Have a minimum of social studies or other relevant qualification such as Healthcare Support – QQI Level 5 is essential
Have the ability to plan and implement suitable activities in partnership with adults
Act as a Key Worker for a group of people attending our services
Have a working knowledge of New Directions policy is desirable
Excellent written and verbal communications skills
Have a good awareness of health & safety issues
Good IT skills to include Excel and Microsoft Office
Be an excellent and innovative team member with a strong commitment to the empowerment of people who have a disability
