Background:
AbbeyBreaffy Nursing is a well-established Nursing Home, 55 beds, located close to Castlebar Co Mayo. It provides a high standard of care which is person centred with the promotion of the well-being of each resident. It is a well establishment highly regarded centre for care for older people and is an integral part of the local community.
AbbeyBreaffy Homecare services now wish to extend their professional and caring services into the community of County Mayo through the provision of a Homecare Service. This service will be provided by professional, highly trained and caring Caregivers who will provide services to support service users, so as they can remain independent and live in their own homes with a high quality of life. The wellbeing of each service user will be central to the care which will be delivered in accordance with the individual needs of each service user agreed in their Care Plan
Job Purpose
To manage and co-ordinate the provision of the AbbeyBreaffy Homecare service to service users who require care and support in their homes.
Job Description
Job Title Homecare Services Manager.
Reports to: Managing Directors AbbeyBreaffy Homecare Services .
Base: AbbeyBreaffy Nursing Home, Castlebar, Co Mayo.
KEY RESPONSIBILITIES:
Manage the Homecare Services in order to ensure that the service is delivered in an appropriate, caring and respectful manner
Main Activities:
Ensure that care is provided according to all relevant policies, procedures and regulations
Ensure that Caregiver staff are recruited and managed to deliver the required care
Monitor and evaluate all tasks of the Homecare Services Caregivers and the program
Monitor supplies and resources
Identify persons requiring home care and service users needs
Liaise with nursing management of Abbeybreaffy Homecare Services nursing resource in relation to care plans, assessments, risk assessments of service users and in relation to clinical needs,issues of concern which may arise from time to time
Make recommendations for changes and improvements to the service, as required
Discuss any concerns with Managing Directors
Provide regular reports to management on the homecare service
2. Coordinate Case Management
Main Activities:
Ensure that all service users needing care are identified
Ensure that current, accurate, confidential client reporting system are in place
Ensure information is provided to other health care professionals, as required
Consult with family members and other supports to ensure that care is on-going and that all client needs are identified and met
3. Provide individualized home care
Main Activities :
Provide Home Care to older persons, persons with disabilities, and others in the community with identified needs
Manage the Provision of Home Care including general housekeeping, personal hygiene and Meals-on Wheels
Manage and organize time and resources based on the individual needs of service users
Manage any unanticipated events or unstable situations
Advocate on behalf of service users for additional service and resources, as required
4. Human Resources Management
Manage and Supervision of Caregivers in the delivery of care services
Recruitment of Caregivers to work in AbbeyBreaffy Homecare Service
Organise training and development of caregivers in line with mandatory and other necessary training
Personal Development
Training & Development:
Maintain professional knowledge and competence
Attend courses and updates as deemed necessary
Attend regular meetings with Directors and other managers
Assist in the training and mentoring of all staff
General Responsibilities
Other duties:
The Manager provides and co-ordinates individual care and resources to service users in their homes and the community.
To manage the Homecare Service regarding adverse events including:
Securing cover for shifts
Confirming weather conditions/adverse occurrences in relevant area
Ensure the completion by Caregivers of good quality written records in relation to personal care, medication and well-being and also reports regarding the service users in our care; as instructed by the Directors of AbbeyBreaffy Homecare and in line with company policies and procedures
Work with Nursing Management in Abbeybreaffy Homecare Services nursing support in relation to clinical areas, risk assessments and any areas of concern in relation to services users care.
Health & Safety:
Manage, implement and comply with Health and Safety legalisation
Report immediately to the Directors any illness, accident or non-entry to a service users home as per AbbeyBreaffy Homecare Service policies and procedures
Promote safe working practices within all service users home
Special Conditions:
This post requires the holder to be available at varying times, which include early morning and late evening work and may involve an on call rota system. The post holder is required to work weekends and bank holidays when required
To carry out other duties and responsibilities commensurate with the post and the needs of the organisation in order to develop and maintain service delivery
The post holder requires the holder to have a clear disclosure check in accordance with regulatory body such as HSE

Knowledge and Qualifications:
Essential:
Qualifications:
Recognised qualification in health related area such as nursing, social care, occupational therapy or other.
Experience/Knowledge:
Demonstrate the ability to make good decisions in relation to the operation of the homecare service and draw up progress reports for Directors on a regular basis
Demonstrate experience in managing a caregiver service
Service User assessment
Staff management and service management evaluation
Knowledge of all relevant legalisation, policies and procedures of AbbeyBreaffy Homecare Services
Good communication and interpersonal skills
Team player
Flexible approach to work according to the needs of the service
Demonstrate motivation, reliability and commitment to team working and the development of others
Ability to maintain accurate and up to date records and task schedules in accordance with company policies and procedures
Desirable:
Relevant Management Qualification
Experience in managing a caregiver service setting
Experience in management of a health service
Experience and certification in moving and handling

Ability to convey information and persuasive skills and deal empathetically with distressed or anxious service users, families and care staff

This is a full time position 40 hours per week, with an on-call commitment for out of hours at night and week-ends
Annual Leave 22 days per annum excluding Public Holidays
