Job Title and Grade: Primary Health Care Coordinator – Primary Health Care for Travellers Project

Closing date: 16th April, 2015

Eligibility Criteria: Public Health Nurse, Registered General Nurse, Community Health Project Personnel, Community Development Personnel or relevant others with a qualification in Health Promotion Community or Public Health and with 3 years experience working in a community setting or with marginalized groups.

Location of Post: Based in Pavee Point, North Great Charles Street.

Organisational Area: Community Health Care Organisation – Area 9 (Finglas/Blanchardstown Area)

Reporting Relationship: The incumbent of this post will report to the Primary Health Care Project Steering Group. The appointment is a joint appointment with Pavee Point and the THU. On a day to day basis the incumbent will have a reporting relationship to their line manager in Pavee Point.

Works With: The Primary Health Care for Travellers Project Steering Committee

Purpose of the Post: To facilitate and support the  promotion of a primary health care model to the Traveller Community through empowerment of Traveller Community Health Workers  in addressing the health status of Travellers in the community.

Principal Duties and Responsibilities:

To jointly coordinate with the Primary Health Care project co-cordinator employed by Pavee Point.
To develop, implement and review needs assessments and training modules for the TCHW’s in conjunction with the joint coordinator.
To develop annual and quarterly work plans in conjunction with all stakeholders towards addressing and improving the health status of the local Traveller Community and responding to their health needs.
To jointly write quarterly and annual reports on the work/ outputs of the project for the Steering Committee and the Regional THU
To collect data and qualitative information to facilitate the ongoing documentation of the process/ outcomes of the PHCTP.
To jointly support and supervise the delivery of  health programmes with and by the Community Health Workers.
To jointly develop new initiatives to respond to the ongoing and emerging needs of the local Traveller community in partnership with the TCHW’s and the Steering Committee.
To jointly support the ongoing development of Culturally      appropriate Health Materials for the PHCTP
To jointly monitor and evaluate the work of the Primary Health Care for Travellers Project on an ongoing basis
To jointly ensure that the project is run according to current models of good practice.
To liaise and work with other relevant statutory and voluntary organizations and agencies as appropriate to the needs of the PHCTP.
To advocate for and facilitate the mainstreaming of Traveller’s within the local health services, through the development of culturally appropriate provision, training and implementation of appropriate codes of practice
To jointly develop and deliver in-service training sessions for health professionals and health trainees to make them aware of the challenges in Traveller Health
To jointly collate statistics and to highlight the role and benefit of the operations of the Primary Health Care for Travellers Project.
To consider the health, safety and welfare of the project staff, in accordance with the Health and Safety at Work Act.
To perform other relevant duties as may be determined by the / The Primary Health Care Project Steering Committee

Requirements

Candidates should be able to demonstrate the following:

Professional knowledge in the area of Health Promotion, Primary Health Care, Community Development and National Strategies with a focus to improving the quality of life of Irish Travellers marginalized groups
Evidence of being a team player and skills in partnership and team working essential
Evidence of effective planning, training and organizing skills
Ability to  manage deadlines and effectively manage multiple tasks
Effective communication skills including the ability to creatively present information in a clear and concise manner, taking cognizance of culture and literacy ability
Ability to facilitate and manage individuals and groups
Evidence of ability to empathise with and treat colleagues and participants  with dignity and respect
Awareness and appreciation of the needs of the community.
Ability to work with multidisciplinary team members
Be self-directed and motivated
Planning, training, leadership, documentation and report writing, research and analytical skills.
