Working in a professional role and environment, core responsibilities include:
Managing a case load to achieve service targets, working as part of a multi disciplinary team.
Implement and work within care and case management systems and protocols.
Working in cooperation and partnership with other agencies, both voluntary and statutory, ensure good communication and cross-agency flexible working practices so as to minimise obstacles to a person getting access to supports, services and/or accommodation.
Engaging with and building a professional trusting relationship with people, treating them with dignity and respect in order to work within the values of Dublin Simon Community.
We are looking for an individual with a degree in a relevant discipline and a minimum of 3 years experience working in a social care setting, who is client centred and committed to providing the highest levels of quality service.
