Project Worker- Outreach – Dublin Simon Community

Minimum job requirements:
BA/BS degree in psychology, social care, addiction, counselling, youth work, community development, psychotherapy or similar.
3 years experience working in a Social Care setting.
Client-focused, adaptable, accountable, resilient individual.
Experience key working; managing a caseload, using harm reduction approaches/initiatives with the ability to advocate on the client’s behalf and record accordingly.
Experience developing strong relationships, with clients and stakeholders.

Preferred job requirements:
Experience working in outreach services and/or with people in active addiction.
Drivers Licence.
Knowledge of accommodation services and entitlements of people who have become homeless.
Passion for helping people, with the ability to work in a constantly changing environment with a problem-solving attitude.
About the job:
As a Project Worker you’ll take the lead on key projects, while working closely with the Service Manager to support and deliver a high quality outreach services. You’ll implement and deliver care and case management with the objective of moving clients into accommodation. You’ll report to the Outreach Manager. As well as our induction program, you’ll complete a key working skills accredited course e.g. DCUtraining and will be provided with 10 training days throughout the year, ensuring you have the support and training to succeed in this role.

Responsibilities:
Develop, manage and implement support plans of 15- 20 clients, while working as part of a multi-disciplinary team, to achieve service and clients targets.
Act as the key liaison on harm reduction with external and internal stakeholders, and ddeliver street based harm reduction interventions.
Conduct assessments on rough sleeper clients; support and engage clients to establish networks that assists them in removing barriers to exiting homelessness; partner with voluntary and statutory agencies.
Provide follow-up support to facilitate access to accommodation (short and long-term), residential treatment and medical services.
Identify and manage risks, ensuring the protection of all clients.
Conduct street searches and identify service-resistant clients.
Promote choice, independence, health and well being.
Drive and implement care and case management processes and protocols.
Provide in-reach and out-reach to rough sleepers, prioritising entrenched rough sleepers.
Induct new team members and carry out relevant on- the-job training and guidance.
Utilise evidence-based techniques such as motivational interviewing to explore clients’ motivation to change.
Responsible for the achievement of local service goals.
Location: Capel Street, Dublin
Working Hours: 7 days liability (Monday – Sunday). 37.5 hours per week. 25 days annual leave.
