Support Coordinator – ALONE

Befriending:
Support older people to identify the cause of their loneliness or isolation and to inclusively work towards accessing the support that will enhance the older persons’ social networks.
Carry out a comprehensive assessment with older people to ensure that all aspects of the older person’s circumstances are supported and that any risks are mitigated.
Support volunteers to work safely and to develop a mutually beneficial relationship with older people availing of our services.
Promote community activation by empowering the older person and volunteer to seek out additional social supports in their area, including ALONE events.
Support Volunteer leaders to guide and support befriending volunteers on an ongoing basis.
Ensure that older people receiving ALONE’s befriending service receive a frequent check in call.
Develop and implement programmes that promote the continuous improvement of ALONE’s befriending services.
Cultivate a positive and supportive atmosphere between the older person, volunteers and staff in ALONE.
Work with ALONE’s HR Dept. to ensure that suitable volunteers are recruited and trained.
Develop and maintain referral pathways to ALONE’s befriending services.

Support Coordination:
Support older people living in the community to identify the issues that may be impeding their quality of life. Adopt an inclusive, person centred approach towards accessing the support that will support the older person’s needs.
Where required, carry out a comprehensive assessment with older people to ensure that all aspects of the older person’s circumstances are supported and that any risks are mitigated.
Manage and lead the response to complex cases including, but not limited to, issues regarding emotional wellbeing, housing issues, health difficulties and vulnerable persons at risk of abuse.
Provide accurate information, advice and advocacy to older people and those supporting older people, through the use of brief interventions.
Maintain necessary confidentiality relating to the work of the service and regarding older people, staff and partner organisations and in line with guidance in the use of the ALONE Management Information System (MIS) and data protection policies and procedures.
Develop and implement programmes that promote the continuous improvement of ALONE’s support coordination services.
Develop and maintain referral pathways to ALONE’s Support Coordination services.
Housing with Support:

Ensure that a full tenant induction is carried out in collaboration with the ALONE Housing Team.
Carry out a comprehensive assessment with older people to ensure that all aspects of the tenant’s circumstances are supported and that any risks are mitigated.
Promote community activation by empowering ALONE tenants to seek out additional social supports in their area, including ALONE events.
Work in partnership with ALONE’s housing department to ensure that ALONE fulfils its role as both a landlord and a care provider.
Maintain necessary confidentiality relating to the work of the service and regarding older people, staff and partner organisations and in line with guidance in the use of the ALONE MIS and data protection policies and procedures.
Develop a network of supports that ensures that no tenant becomes isolated or is at risk of vulnerability. Part of this network will entail a 5-12 week check in call to ensure that all needs of tenants are assessed and supported.
General Requirements for all ALONE staff:

Work to enable older people to remain at home with quality of life and independence.
Attend all meetings and training as requested.
Work professionally and confidentially and in line with ALONE’s rights and evidence based approach.
Keep all records, data and reports up-to-date and make these available upon request.
Participate in the development and implementation of all Quality Management System documents.
Participate in the efficient flow of information within the organisation by sharing and seeking information as appropriate.
Participate in and engage with the performance management programme.
To manage and deliver action plan items in line with ALONE’s strategic plan.
To undertake any other duties requested by the Manager and or CEO.

Person Specification:
Essential:
Educated to degree level 7 or above in a relevant discipline
Minimum of 1 years’ experience of working in a social care setting.
Self-motivated person who is passionate about the work.
Excellent inter-personal skills and experience of delivering presentations.
Excellent computer literacy & ability to adapt to new systems.
An ability to maintain the highest standards of confidentiality and respect.
A full clean driving licence and access to own vehicle is required.
Must be flexible and available to do evening and weekend work.

Desirable:
Knowledge of case management processes and competencies.
Experience of volunteer management and older person’s services.
An understanding of the benefits and challenges of adopting a multi-disiplinary approach towards the care of older people.
Experience of management information systems and the role of case management therein.
An understanding of the principles and approach to community activation.

Position:
Fulltime position (35 hours Monday – Friday), initially one year contract.
Reporting to the Support Services Manager.
Place of work: This position will be based in Dundalk, Co. Louth
Travelling within the surrounding counties may be required
