The successful candidate must have
Current registration with Nursing Board of Ireland
Evidence of providing individualised supports and commitment to community living
Experience and proven track record of working successfully to support individuals and their families
Excellent verbal and written communication skills
Excellent up to date clinical skills related to health promotion and well being
Experience in self direction, planning and implementing a support package to meet a persons’ individual needs in a community based inclusive manner
An understanding of primary care and specialist health services available in community
Commitment to developing a new service delivery model
Full drivers’ licence
Desirable Experience
Networks and contacts with community partners and organisations
Training in using the ‘Supported Self Directed Living’ (SSDL) theory of practice, or willing to undertake such training
A good knowledge of medications management and interest in train the trainer medication management delivery
Experience of supervision, mentoring and delivery of training
Flexibility of work location and hours of work are requirements of the post within the Cheeverstown catchment area.
Salary will be reflective of qualification and level of experience as per Dept. of Health & Children salary scales
