PROFESSIONAL & ETHICAL PRACTICE Under the direction of Senior Management, the Health Care Co-Ordinator will Practice nursing according to: The Professional Code of Conduct as laid down by the Nursing and Midwifery Board of Ireland Professional Clinical Guidelines Dara Residential Services policies, protocols and guidelines Current legislation as it applies to the role including National regulations The Health Care Co-Ordinator will be responsible for oversight and co-ordination of health care for persons supported with a diverse range of health care needs. Assess, plan, implement and evaluate each persons best possible health, health action plan, health profiles in accordance with best practice and grounded in recent evidence based nursing research. Develop and maintain close links with the people supported, frontline staff, families and day services ensuring a partnership approach. Work closely with the staff and management team to achieve optimal input for each person supported Maintain appropriate and accurate written records and reports regarding persons supported best possible health in accordance with local and national guidelines. Foster a philosophy of care that reflects Dara Residential Services commitment to quality, using an evidenced based approach, in a safe environment maintaining the dignity of the each person supported. Monitor overall healthcare, prevention of illness, and promote healthy lifestyles with persons supported and respite users. Support and/or facilitate relevant healthcare appointments planned and emergency. Ensure national screening programmes are availed of by people supported by Dara Residential Services Residential Service and Individualised Residential Service. Act as an advocate for people supported and their families and provide information to empower them in decision making, goal setting and problem solving in order to optimise their contribution and facilitate their participation in community based activities. Promote and maintain nursing standards of person centred care, including the promotion of normalisation and quality of life issues. Participate in the development of protocols and guidelines, and the setting of professional nursing standards �?� Be competent in relation to all relevant clinical policies and procedures �?� Participate in service audits including medication audits and health review audits �?� Comply with all legal requirements pertaining to support of people with Intellectual Disabilities �?� Encourage persons supported engagement in community activities and escort and participate in such activities thus promoting a socially inclusive model of living. �?� Contribute to, implement, monitor and review the development of risk assessments for persons supported �?� Professional Expectations: The Health Care Co-Ordinator must comply with the Code of Professional Conduct as laid down by the Nursing and Midwifery Board of Ireland. The nursing profession demands a high standard of professional behaviour from its members and each registered nurse is accountable for his or her practice. The nurse must work within his / her scope of practice and must take measures to develop and maintain the competence necessary for professional practice. The nurse must be aware of ethical policies and procedures which pertain to his / her area of practice. Service users confidentiality must be maintained at all times. Have a working knowledge of the Health Information and Quality Authority (HIQA) Standards as they apply to the role for example, Standards for Healthcare, National Standards for the Prevention and Control of Healthcare Associated Infections, Hygiene Standards etc. EDUCATION & STAFF DEVELOPMENT: The Health Care Co-Ordinator will: Participate in the planning and implementation of orientation and training programmes both for frontline staff and frontline managers in health related areas. Provide education and information to persons supported / family as required. Take responsibility for own learning. ADMINISTRATION/ MANAGEMENT: The Health Care Co-Ordinator will: Accept overall responsibility for and co-ordinate the management of healthcare for each person supported by Dara Residential Services. Take management responsibility for staff replacement on rosters when required. Provide support and give feedback to the frontline management team. Ensure compliance with national regulations, health and safety legislation and risk management policy. Participate in the development of policies within these areas in conjunction with relevant staff. Take responsibility for the auditing and reviewing of all healthcare systems including medication management policy and procedures.

Skills Requirements
Be on the current register as maintained by Nursing and Midwifery Board of Ireland and preferably possess the RNID qualification. Have experience of HIQA, inspection process and regulatory obligations. Excellent facilitation, communication skills, report writing and IT skills. Full Clean Drivers Licence. Evidence of continued commitment to professional development .
