Primary Healthcare Nurse – Physical Health – Dublin Simon Community

The Role
An exciting opportunity for a Primary Healthcare Nurse- Physical Health who will work across a number of Dublin Simon Community Housing Services. He/she will be responsible for oversight and co-ordination of health care services for clients with a diverse range of health care needs. The role includes assessment of the health needs of clients which addresses their physical, social, psychological cultural and health needs. This will entail effectively co-ordinating and liaising with health care providers in the community to ensure that clients are receiving the right care at the right time by the right person in a safe caring environment that leads to the best possible healthcare outcomes.

This role is rewarding and varied allowing the individual to use their skills and knowledge to ensure effective outcomes for our clients.

Working in a professional role and environment, core responsibilities include:

Conduct nursing assessments with service users to establish their health care deficits, attend weekly case meetings and work as part of a multi-disciplinary team to support service users to achieve outcomes as set out in their support plans/individualised nursing care plans
To liaise with , a range of voluntary and statutory service providers to establish, expand and maximise the range of services available to clients in Dublin Simon Community supported housing services, to include but not limited to, public health nurses, physiotherapists, GP’s , Occupational Therapists, Addiction Treatment Services, Mental Health Teams, STD clinics, Tissue Viability Nurses etc. To establish referral pathways where required.
Facilitate groups through an effective health education and promotion programme in order to ensure that service users receive relevant information for making informed decisions about their future.
Record key performance indicators and deliver on targets, demonstrating the health improvements of residents.
Be responsible for implementation of Infection Prevention and Control-Standard Precautions Guidelines in supported housing services.
Liaise with hospitals on discharge planning and perform home assessments prior to discharge from hospital or other institution where need identified.
We are looking for an individual with RGN (BSc Nursing) with evidence of PIN renewal for 2016. We require that you have a minimum of 2 years post registration experience, with an understanding of why people become homeless and their needs. You will also have an understanding of physical and mental health, drugs, alcohol and other issues relevant to homeless service users with knowledge of relevant government policies relating to homelessness, health and addiction issues.
